Implementation sequence:
Employees submit documents to the unit listed in the components and number of documents below.
Employing units
1. Instruct social insurance and health insurance participants to fill out the declaration, check, compare and sign the declaration of the employee.
2. Submit documents according to the regulations listed in the components and number of documents below, electronic data (if any) at the one-stop department of the social insurance agency.
For social insurance agencies
1. One-stop department: Receive documents, electronic data (if any) of the unit. Count the components and quantity, if correct and sufficient according to regulations, write an appointment letter. In case the documents are not correct and sufficient according to regulations, clearly state and return them, then transfer the documents to the Collection Department.
2. Collection Department: check, compare records, enter data into the collection management program. Transfer processed records to the Book and Card Issuance Department.
3. Book and Card Issuance Department: Check, compare records, issue social insurance books (for cases where there is no social insurance book), health insurance cards and transfer the settlement results to the one-stop department.
How to do it: Submit directly to the one-stop department, electronic data can be transferred via the internet.

• Unit:
1. Declaration of unit participation, adjustment of social insurance and health insurance information (Form TK3-TS)
2. List of employees participating in social insurance, health insurance, unemployment insurance, occupational accident and disease insurance (Form D02-TS).
3. Information list (Form D01-TS).
• Employees:
1. Declaration of participation, adjustment of social insurance and health insurance information (Form TK1-TS).
2. For those who are entitled to higher health insurance benefits (people with meritorious services...): provide additional supporting documents (if any).
Processing time: No more than 05 working days from the date of receiving complete documents as prescribed.
Subject of implementation: Organization.
Implementing agency: Provincial, District Social Insurance
Results of administrative procedure implementation: Social Insurance Book, Health Insurance Card.
